Verify tests and prepare the certificate for a new three phase supply

What has happened
Job J-26-1147 at Fairmont Engineering Works in Geelong installed a new 63 A three phase supply and isolator for a CNC machine. The Leading Hand completed the tests on Friday and the compliance register shows the results as Tests recorded. The General Manager checks every prescribed work certificate before it is lodged and has asked for a review of the test sheet against the policy and procedure before he signs. The customer wants the machine running on Wednesday. The test sheet shows earth continuity, insulation resistance, polarity and fault loop impedance, but the residual current device test line is blank and the instrument identification is written as 'van tester'.

Evidence guide
The student must identify the missing residual current device test and the inadequate instrument record, and must not accept a pass without measured values. Look for correct sequencing of tests, an instrument identification traced to the register with current calibration, and a certificate that matches the work and the recorded results. The note to the General Manager should be factual and brief.
